ACCESS TO MEDICAL RECORDS
The practice is registered and complies with the Data Protection Act 1998. Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act. Please contact the Practice Manager for further information.
Your Data Matters to the NHS
Your health records contain a type of data called confidential patient information. This data can be used to help with research and planning.
You can choose to stop your confidential patient information being used for research and planning. You can also make a choice for someone else like your children under the age of 13.
Your choice will only apply to the health and care system in England. This does not apply to health or care services accessed in Scotland, Wales or Northern Ireland.

Access to health records
In general you have the right to see your health records though certain legal exclusions may apply. You can ask to see your record and may request a copy of any part subject to a small administrative charge. If you require access to your records please ask to speak to the Practice Manager, in the first instance. You may also wish to sign up to the on line access to your records, and test results etc. Please contact the surgery for more information.

If you have any concerns regarding the confidentiality of your personal medical records, or you would like further information, please do not hesitate to discuss this with your doctor, or the Practice Managers, Teresa Cawsey or Ann Loxton.
