NON NHS SERVICES

Not all the services we provide are paid for by the National Health Service .  We recommend that you contact the surgery so we can advise you of likely charges. Most will depend on the length of time the doctor will spend completing the information, and whether an examination is needed.

The types of services that are not covered by NHS are, for example:

• Letters (ie to whom it may concern)
• Adoption/Fostering and childminder/Ofsted forms
• DVLA, Taxi and Sports examinations and forms and fitness statements
• Medical reports for insurance companies, solicitors, employers etc
• Private sick notes and health care reports
• Holiday cancellation form or certificate
• Power of attorney forms
