Visiting the surgery and collecting a registration form
You can register at the practice by bringing your medical card to the surgery or signing and completing a registration form which we will supply.
If you cannot come to the surgery, registration may be carried out by an authorised representative who may sign the registration on your behalf.
Please note that we only accept patients who are entitled to receive NHS treatment and whose permanent address lies within our practice catchment area.
Patients are registered with the practice and may consult any of the doctors but we prefer the same doctor to deal with each episode of illness where possible.
Temporary residents and visitors
Visitors to the area are welcome to register as temporary patients of the surgery.
Please note that only patients entitled to NHS services can register for free treatment. Those who are not may be seen as private patients. Please ask reception for details.
New patient health check
When you register with the practice you may wish to make an appointment with a nurse for a new patient health check. This allows any health problems or family history to be recorded prior to the receipt of your medical record and allows you to discuss any issues or concerns you may have.
