HOW TO REGISTER
Welcome to Grove House Surgery
We are pleased to accept registration requests from patients who are living within the area:
Please see our practice boundary and check your postcode is within our catchment area.
You can complete some of the forms online and submitt electronically to the surgery (see below).
OR If you prefer, you can collect forms from the Surgery.

NB: If you cannot come to the surgery, registration may be carried out by an authorised representative who may sign the registration on your behalf.
Please note that we only accept patients who are entitled to receive NHS treatment and whose permanent address lies within our practice catchment area.
Patients are registered with the practice and may consult any of the doctors, but it is preferable to follow up incidencences of the same illness with the same doctor where possible.
Temporary residents and visitors
Visitors to the area are welcome to register as temporary patients of the surgery.
Please note that only patients entitled to NHS services can register for free treatment.
New patient health check
When you register with the practice you may wish to make an appointment with a nurse for a new patient health check. This allows any health problems or family history to be recorded prior to the receipt of your medical record and allows you to discuss any issues or concerns you may have.
