Patient confidentiality
We ask you for personal information so you can receive appropriate care and treatment. This information is recorded on computer and we are registered under the Data Protection Act. The practice will ensure that patient confidentiality is maintained at all times by all members of the practice team. However, for the effective functioning of a multi-disiplinary team it is sometimes necessary that medical information about you is shared between members of the team. Access to confidential patient information is given only to those surgery staff who need it in order to provide a service to the patient. All staff working in the surgery are subject to stringent confidentiality requirements and no information from your record will be released to third parties outside the NHS without your specific consent.

Your health records contain a type of data called confidential patient information. This data can be used to help with research and planning. You can choose to stop your confidential patient information being used for research and planning. You can also make a choice for someone else, like your children under the age of 13. There are strict rules about how NHS can use your data. It’s only shared securely and safely. If you are happy with your confidential patient information being used for research and planning you do not need to do anything. Any choice you make will not impact your individual care.
